The Community Health Program is here is assist Caddo tribal members with their health needs. The Program is funded by the Indian Health Service on a yearly basis. The CHR Program's purpose is to assist eligible Indian individuals and families within our service are to gain access to comprehensive health care services.
All CHR staff members are trained in CPR, First Aid and EMS First Responder.
The CHR Program is often a vital link to health care services for our Indian people in rural areas. Our work includes making and rescheduling appointments for clients, pick up and delivery of medications, home visits, case screening and management, monitoring of patients, and health education.
The staff transports clients who are without transportation to doctor's appointments at the Anadarko IHS clinic or the Lawton IHS hospital. We can also provide transportation to outside referrals made by a doctor from IHS. The CHR Office needs to know 2 days to 1 week in advance about appointments, refills of prescriptions, and transportation needs. There is a call-in procedure for requesting medication at the Lawton Indian Hospital; call toll-free 1-888-275-4886 for assistance.
Bonnie Stephenson, the CHR Director also handles Tribal Health Services, which provides assistance up to $100 for eyeglasses, dentures, health equipment, cost of prescriptions, and hearing aids. Assistance can be provided to individuals once per year.
Ms. Stephenson also administers the Caddo Nation's Diabetes Fund through the CHR Office. Funds are available to assist tribal members who have diabetes with eyeglasses, dentures, hearing aids, and medical supplies. Assistance can be provided to individuals once per year.
The Caddo Tribal Council appropriated the annual funds for Health Services in January to provide assistance to enrolled tribal members in the following categories: eye glasses and contacts, hearing aids, dental work, prosthesis, prescription and medical bills, and health equipment. Assistance is a one-time only, per year maximum of $100 for enrolled tribal members. Applications for assistance may be obtained at the Tribal Office or the Community Health Office and returned with a bill attached to the Health Department. The Anadarko Indian Clinic is striving to offer the best care for the prevention of foot amputations and complications of open wounds by offering state of the art wound care for patients. Would care specialists and a physician are available to assist you. This service is available every Thursday from 8 AM to 4:30 PM.
